What You Need to Prepare:

- Copy of the patient’s passport bio-page
- Patient’s WhatsApp-enabled contact number (or alternative, such as your own)
- Patient’s active email address (or your email as backup)
- Medical reports in English
- If your reports are not in English, kindly request an official translation from the issuing hospital or laboratory
- Please avoid using AI translation tools.
- If translation isn’t available in your country, Icon Cancer Centre can provide a professional medical translator (additional fees apply)
